During internal investigation, the manufacturer determined that the particle originates as result of abrasion of the gaskets used as part of a test device in production where each vacuum-tight version of a Venous Hardshell Cardiotomy Reservoir is tested for over pressure, vacuum and tightness. It was found that the abrasion was caused by a too narrow diameter of the gasket.
